Online health risk assessments — quizzes, calculators, "longevity scores" — are everywhere, and it's fair to wonder how much to trust them, especially when different tools give different answers for the same person. Here's an honest look at what these tools can and can't do, and why so much longevity advice online seems to contradict itself.
What online risk assessments are actually built on
Most consumer health and longevity assessments work the same way: you answer questions about your age, weight, activity level, sleep, diet, and sometimes symptoms, and an algorithm compares your answers to population-level risk data to estimate your relative risk of certain conditions.
This can be genuinely useful as a directional tool — it can reasonably flag "your reported habits and age put you in a higher-risk category for X" in a way that's worth taking seriously. What it cannot do is replace an actual measurement. A questionnaire asking whether you feel your blood pressure is "usually normal" is fundamentally different from a doctor actually measuring your blood pressure, and the same gap applies to blood sugar, cholesterol, and most other markers that matter most (see blood markers that predict health).
Where the accuracy genuinely breaks down
A few specific weaknesses are worth understanding:
- Self-reported data is unreliable. People routinely misjudge their own activity level, sleep quality, or diet, which shifts the output without the tool ever knowing.
- No test data means no real risk stratification. Two people who answer identically but have very different actual blood pressure or cholesterol will get the same output, because the assessment never saw either number.
- Population averages don't account for your specific family history in any detailed way, beyond a checkbox for "family history of X."
- Different tools weight factors differently, which is a large part of why the same person can get meaningfully different scores from different apps — there's no single, universally agreed formula behind "longevity scores."
None of this makes these tools worthless. It makes them a reasonable starting point for a conversation, not an endpoint or a diagnosis.
A reasonable way to use one
If you've taken an online risk assessment and it flagged something, the useful next step is rarely to take three more quizzes hoping for a clearer answer — different tools' different weighting means you may just get a third, equally uncertain number. Instead, treat the flag as a reason to get the actual measurement done: an HbA1c if sugar risk was flagged, a blood pressure check and lipid panel if heart risk was flagged, and so on. The quiz did its job if it got you to book that test; it wasn't designed to replace it.
Why longevity advice online so often contradicts itself
This is a genuinely common, reasonable frustration, and it has a straightforward explanation: online content mixes several different levels of evidence and presents them with roughly the same confidence.
- Established clinical guidelines — built on large studies and expert consensus — sit alongside small preliminary studies, animal research, and individual anecdotes, often with no indication of which is which.
- The same finding gets framed differently by different sources — a modest, well-established benefit from one source can appear alongside another source's exaggerated version of the same underlying research, described as if it were a bigger or more certain effect than the evidence actually supports.
- Content optimised for engagement tends to favour bold, simple claims over the more accurate, more boring "it depends on your specific situation."
- Advice genuinely differs by individual — a recommendation that's right for someone with normal blood pressure can be wrong for someone with hypertension, but generic content rarely carries that caveat clearly.
- Commercial incentives shape a meaningful share of longevity content, since it's frequently attached to a product or programme being sold.
Once you know this, the contradictions make more sense — they're less often "someone is lying" and more often "these two pieces of content are answering different questions, for different people, with different quality of evidence behind them."
A practical way to sort through it
Rather than trying to resolve every contradiction yourself, a few filters help:
- Favour content that cites established clinical guidelines or large studies over ones that lean on personal testimonials or a single small study.
- Be more skeptical of anything promising fast, dramatic, or guaranteed results — legitimate preventive medicine tends to describe modest, gradual, evidence-based improvement, not transformation.
- Notice whether advice acknowledges it depends on your specific situation — content that's honest about its own limits is generally more trustworthy than content that applies one rule to everyone.

The most sensible way to use an online risk assessment is as a nudge, not a verdict: if a quiz flags elevated risk in a category, treat that as a reasonable prompt to get the actual test done and discuss it with a doctor — not as confirmation of a diagnosis, and not as something to dismiss either just because a different app gave a different score. The tool's job is to get you to ask the right question. A doctor's job, using your actual numbers, is to answer it.
