TSH tells your doctor how your thyroid is functioning right now. Antibody tests tell a different story — why it might be changing at all.
If your doctor has ordered an anti-TPO test alongside your usual TSH, it's not a routine add-on — it's checking for a different thing entirely: whether your immune system is involved.
What anti-TPO actually tests for
Anti-TPO stands for antibodies against thyroid peroxidase, an enzyme your thyroid needs to make its hormones. When your immune system mistakenly produces antibodies against this enzyme, it's attacking your own thyroid tissue — this is what's meant by an autoimmune thyroid condition, most commonly Hashimoto's thyroiditis.
TSH measures how your thyroid is functioning right now. Antibody tests measure whether an autoimmune process is driving that function — a different, earlier layer of the picture.
What a positive result means
A positive anti-TPO result confirms an autoimmune process is present. On its own, it doesn't tell you how your thyroid is currently performing — you can have positive antibodies with completely normal TSH, particularly early on. In that case, your doctor may suggest periodic monitoring rather than starting treatment immediately, since the antibody presence flags a higher chance of thyroid function changing over time, not a certainty that it will.
Other thyroid antibodies your doctor might check
Anti-TPO is the most commonly ordered thyroid antibody test, but it isn't the only one:
- Anti-thyroglobulin (anti-Tg) antibodies — often checked alongside anti-TPO in autoimmune thyroid disease. Less commonly used alone for diagnosis, but relevant if thyroglobulin is being tracked for other reasons.
- TSH receptor antibodies (TRAb), including a specific type called TSI — these are checked when Graves' disease, the autoimmune cause of an overactive thyroid, is suspected. Unlike anti-TPO, these antibodies can directly stimulate the thyroid rather than simply mark autoimmune activity.
Which antibody test gets ordered depends on whether your doctor suspects an underactive process (Hashimoto's, where anti-TPO is the workhorse test) or an overactive one (Graves', where TRAb matters more). Bringing up your specific symptoms — cold versus heat intolerance, weight change in either direction — helps your doctor choose the right one rather than ordering all of them by default.
What a Hashimoto's diagnosis actually involves
A Hashimoto's diagnosis isn't based on antibodies alone. Your doctor typically looks at three things together: your TSH and Free T4 results (how your thyroid is currently functioning), your anti-TPO result (whether an autoimmune process is present), and sometimes an ultrasound of the gland itself, which can show a characteristic appearance in longstanding autoimmune thyroiditis. All three pointing the same direction gives a confident diagnosis; any one alone is suggestive but not conclusive.
Once diagnosed, Hashimoto's is typically a lifelong, manageable condition rather than one that resolves — most people are monitored with periodic TSH checks and treated if and when their thyroid function actually declines, which for some people happens gradually over years rather than all at once.
Should antibody tests be repeated?
Generally, no — once a positive anti-TPO result has been confirmed, most doctors don't see value in repeating the antibody test itself regularly, since the antibody result rarely reverses and doesn't need to be "watched" the way TSH does. What gets monitored going forward is thyroid function (TSH, Free T4), not the antibody level. An exception is sometimes made during pregnancy planning or early pregnancy, where a documented antibody status can inform how closely a doctor wants to follow you.
How common this actually is
In an eight-city Indian study, roughly 1 in 5 adults tested had positive anti-TPO antibodies — a common finding, not a rare or alarming one on its own. It becomes clinically significant in combination with your TSH result and your symptoms, not as an isolated number.
What your doctor does with this information
- Normal TSH, positive antibodies: often monitored periodically rather than treated immediately.
- Abnormal TSH, positive antibodies: treatment decisions follow your TSH result, with the antibody result explaining why — useful for understanding your condition's likely course.

Why "autoimmune" doesn't mean what people fear it means
Hearing that your body is "attacking itself" can sound alarming, but in the context of thyroid antibodies, it describes a slow, usually gradual process rather than a sudden or dangerous one. Many people live for years with positive antibodies and completely normal thyroid function, monitored periodically rather than treated. The antibody result is best understood as information that helps your doctor anticipate what might happen over time, not a description of an active crisis.
What it means for your family
Autoimmune thyroid conditions have a genetic component, meaning a positive antibody result in you can be relevant information for close blood relatives, particularly if they have their own thyroid symptoms or an unexplained TSH result. This isn't a reason for family members to rush out and get tested without cause, but it is worth mentioning to a doctor if a parent, sibling, or child later develops symptoms that could be thyroid-related — family history of autoimmune thyroid disease is a detail doctors specifically ask about for exactly this reason.
