A PMOS blood report can look like a wall of unfamiliar abbreviations. Here is what the ones that actually matter are checking for.
Getting a blood test report back with rows labelled LH, FSH, AMH, and half a dozen other abbreviations is disorienting, especially when nobody has explained what any of them mean. PMOS — the condition most people know as PCOS or PCOD — is diagnosed through a pattern across several tests and symptoms together, not a single number. Understanding what each test is actually checking makes the conversation with your doctor far more useful.
Why PMOS needs several tests, not one
There is no single "PMOS test." The condition is diagnosed by combining your period history, physical signs (like acne or excess hair), and a set of blood tests that together build a picture of your hormone and metabolic pattern. This is why a doctor typically orders a small panel rather than a single test, and why one abnormal-looking number on its own rarely means much without the rest of the picture. The reproductive hormones: LH and FSH
LH (luteinising hormone) and FSH (follicle-stimulating hormone) are both released by the brain and control the ovarian cycle — FSH helps eggs mature, and a surge in LH triggers ovulation. In PMOS, LH is often raised relative to FSH, a pattern sometimes referred to as a raised "LH:FSH ratio." This pattern is common but not universal in PMOS — plenty of women with the condition have a normal ratio, and having a raised ratio alone, without other signs, doesn't confirm a diagnosis. It is one data point a doctor weighs alongside everything else, not a stand-alone verdict.
Testosterone and other androgens
Testosterone and related "male-type" hormones (androgens) are naturally present in everyone, in smaller amounts in women. In PMOS, these are often modestly raised, which is part of what drives symptoms like acne, excess hair growth, and scalp hair thinning (see our PMOS acne, hair loss, and facial hair guide for how that connects). A doctor typically checks total testosterone and sometimes other related markers, since androgen levels help confirm the hormonal pattern behind visible symptoms.
AMH (anti-Müllerian hormone)
AMH reflects the number of small, developing follicles in the ovaries at a given time. Because PMOS involves a larger number of small follicles than typical, AMH levels are often higher in women with the condition. It has become a popular test partly because it requires no specific cycle-day timing, unlike some other hormone tests. Important caveat: a raised AMH suggests a PMOS-consistent pattern but does not, by itself, confirm the diagnosis — some women with genuinely normal ovaries have higher AMH for other reasons, and some women with PMOS have AMH within a normal range. It's a supporting piece of the picture, read alongside your period history and other results.
Insulin and blood sugar markers
Given how central insulin resistance is to PMOS (see PMOS and weight gain), a doctor typically also checks fasting blood sugar, and sometimes fasting insulin or a glucose tolerance test, to understand your metabolic picture. These results matter for two reasons: they help confirm the metabolic side of PMOS, and they flag whether you're at higher risk for related conditions like prediabetes or type 2 diabetes — which is exactly the long-term risk that metabolic-first PMOS care is designed to catch early.
Thyroid tests
A doctor often checks thyroid function (typically TSH) alongside PMOS-specific tests, not because thyroid problems cause PMOS, but because thyroid dysfunction can cause similar symptoms — irregular periods, weight changes, mood shifts — and needs to be ruled out or identified as a separate, coexisting issue. This is a standard part of a thorough workup, not a sign that something unusual is suspected.
Why your report needs a doctor to read it, not a chart
Reference ranges for these hormones vary by lab, by the specific assay used, by your age, and by where you are in your cycle when the blood was drawn — which is exactly why quoting a fixed "normal range" here would be misleading rather than helpful. A number that looks high on one lab's report might be normal for a different lab's testing method, or normal at a different point in your cycle. This is the practical reason a printed report, without a doctor's interpretation, can cause more anxiety than clarity: the same number can mean different things depending on context the report itself doesn't show.
How to make the most of a results conversation
When you go through your report with a doctor, it helps to ask specifically:
- "Which of these results actually point toward PMOS, and which are there to rule other things out?"
- "Is anything here borderline, and if so, what would you want to check next?"
- "Does anything here change what we do about my specific symptoms?"
These questions turn a results review from a pass/fail moment into a useful conversation about what actually happens next.
Timing matters more than people expect
Several PMOS-related hormone tests are meaningful only when drawn at a specific point in your cycle — some are typically done early in the cycle, for instance, while others matter less about timing. If your periods are already irregular, which is common with PMOS, your doctor may adjust how and when tests are done, or interpret results with that irregularity in mind, rather than expecting a textbook-timed sample. This is one more reason a home test or a test ordered without a doctor's guidance can produce a result that's technically accurate but hard to interpret correctly without the full context.
What repeat testing is actually for
Some women are told to repeat certain PMOS blood tests after a few months, which can feel like being sent back to square one. In practice, repeat testing usually serves one of a few specific purposes: confirming a borderline result that could have been affected by timing, checking whether a management approach (diet, movement, or medication) is having the intended effect on insulin or hormone markers, or monitoring a value your doctor flagged as worth watching. It's reasonable to ask directly what a specific repeat test is checking for and what result would change your plan — a good doctor will have a clear answer, because repeat testing without a specific purpose isn't good practice.
A PMOS blood report is not a verdict to decode on your own. It's a set of clues that only make sense read together, with someone trained to read them.
