"I barely add salt while cooking" is one of the most common — and most incomplete — things patients tell their doctor about their diet.
Salt has an outsized reputation problem in Indian kitchens: it's treated as a cooking choice, something you can simply add less of, while the far larger sources sit quietly in a jar of pickle, a stack of papad, and a shelf of packaged snacks nobody thinks to count. A population survey of over 1,395 people across urban and rural India, published in the Journal of the American Heart Association (2017), found average daily salt intake between 8.6 and 9.5 grams — nearly double the 5-gram limit recommended by the WHO, the ICMR-NIN, and India's own National Programme for Prevention and Control of Non-communicable Diseases.
Why salt and blood pressure are linked
Sodium — the component of salt that matters physiologically — causes the body to retain more water, which increases blood volume. More volume moving through the same-sized blood vessels means more pressure against artery walls. Over time, and in people with a genetic sensitivity to sodium (which varies from person to person), this translates into measurably higher blood pressure. It's one of the more directly mechanical relationships in cardiovascular health: more sodium, generally, more pressure.
This is also why sodium reduction shows up consistently in blood pressure management guidance, including the 2017 ACC/AHA hypertension guideline (Whelton et al., Hypertension, 2018), as one of the lifestyle factors doctors discuss alongside — not instead of — medical evaluation and, where indicated, medication.
Where the sodium actually comes from
Table salt added while cooking is only part of the picture, and often not the largest part. In a typical Indian diet, sodium arrives from several directions at once:

The salt shaker gets the blame. The pickle jar rarely does.
Someone who cooks with modest salt but eats pickle daily, snacks on packaged namkeen, and orders in twice a week can easily exceed the recommended limit without ever consciously "adding too much salt" — which is exactly why dietary recall alone often underestimates real intake, and why sodium reduction advice needs to be specific rather than a vague instruction to "cut down."
What actually moves the number
Meaningful sodium reduction rarely comes from one dramatic change. It tends to come from several smaller, sustainable ones:
- Treating pickle and papad as occasional additions, not daily defaults
- Checking packaged food labels for sodium content, not just calories
- Cooking at home more often, where sodium is at least under your own control
- Using herbs, citrus, and spices to build flavour rather than defaulting to more salt
- Being mindful of restaurant and delivery food frequency, where sodium control is out of your hands entirely
None of this requires eliminating salt — sodium is a genuine physiological requirement, not something to remove entirely. The goal is bringing total intake down toward the recommended range, not toward zero.
Reading a label, and what to look for on a menu
Packaged food labels in India list sodium content, but the number rarely means much without a reference point — 400mg of sodium per serving sounds abstract until you know that's already close to a tenth of the recommended daily limit in a single snack. A rough, useful habit: treat anything above roughly 400mg of sodium per serving as high, and be aware that a "serving" on the label is often smaller than what people actually eat in one sitting, which understates real intake further. Restaurant and delivery food rarely lists sodium at all, which is precisely why frequency — not just quantity per meal — is the more practical lever to manage.
Potassium works in something like the opposite direction to sodium, helping the body manage fluid balance and blood pressure more favourably. Vegetables, fruit, dal, and other whole foods common in Indian home cooking are naturally potassium-rich, which means a diet built around more home-cooked, whole-food meals doesn't just lower sodium by default — it also raises potassium intake at the same time, a combination that tends to matter more for blood pressure than either change alone.
Where this fits into a broader evaluation
Salt reduction is one lever among several — alongside weight, activity, alcohol, and where indicated, medication — and its impact varies by individual. Some people are more "salt-sensitive" than others, meaning their blood pressure responds more dramatically to sodium changes; others see a smaller effect.
