Cardiac Tests Explained: Stress Test, Echocardiogram, and Catheterization

A stress test, an echocardiogram, and a catheterization measure different things. Here's what each involves, why it's ordered, and how to prepare.

You've been told to "get a stress test" or "get an echo done" and nobody explained what either actually involves — or why one wasn't enough on its own.

Cardiac testing isn't one test that gets more thorough as your doctor gets more worried. It's a set of genuinely different tools, each built to answer a different question about how your heart is functioning, and doctors choose between them — or combine them — based on what they're actually trying to find out. Understanding what each one measures makes the whole process considerably less opaque.

How doctors decide what to test for

Before any test gets ordered, a doctor is usually working from a specific question: is there reduced blood flow to the heart muscle during exertion (a stress test question), is the heart's pumping function or valve structure normal (an echocardiogram question), or is there a blockage in a specific artery that needs to be seen and possibly treated directly (a catheterization question)? Your symptoms, risk factors, and any prior results shape which test, or sequence of tests, makes sense — which is why two people with similar-sounding chest discomfort can walk out of a consultation with completely different test orders.

The cardiac stress test

A stress test evaluates how your heart performs under physical demand, because some blockages that cause no problem at rest become apparent only when the heart is working harder and needs more blood flow. In the standard version, you walk on a treadmill at increasing speed and incline while your heart rate, blood pressure, and ECG are continuously monitored. If you're unable to exercise — due to joint issues, mobility limitations, or other reasons — a medication that simulates the cardiac effects of exercise can be used instead, sometimes paired with imaging (a nuclear or echocardiographic stress test) for a more detailed picture.

What the test is looking for: ECG changes, chest symptoms, or abnormal blood pressure responses that appear specifically under exertion and that a resting ECG, taken while you're sitting calmly, would never reveal. A completely normal stress test is reassuring; an abnormal one doesn't automatically mean a blockage requiring intervention — it's a signal that guides what gets checked next, per the 2012 ACCF/AHA guideline on stable ischemic heart disease.

The echocardiogram

An echocardiogram is an ultrasound of the heart — sound waves, not radiation, producing a real-time moving image of the heart's chambers, valves, and walls. It's painless, involves no needles, and typically takes 20 to 40 minutes. A technician moves a probe across your chest while images and measurements are recorded.

What it reveals includes the size and thickness of each heart chamber, how well the heart muscle contracts and relaxes, whether any valve is leaking or narrowed, and — critically — the ejection fraction: the percentage of blood the left ventricle pumps out with each heartbeat. An echocardiogram is also the standard way to evaluate a heart murmur properly, rather than guessing at its cause from the sound alone.

Cardiac catheterization

This is a more involved procedure, usually reserved for when non-invasive tests (stress test, echo, CT) have already suggested a specific concern that needs direct visualization or, in some cases, direct treatment. A thin catheter is threaded through a blood vessel — typically in the wrist or groin — up to the heart, where contrast dye is injected so that X-ray imaging can show exactly where, and how severely, an artery may be narrowed. It's a genuinely useful diagnostic and treatment tool, and it also carries real procedural risks, which is exactly why your doctor weighs it against less invasive options first, per the 2021 ACC/AHA/SCAI coronary revascularization guideline.

Preparing for a catheterization

Preparation instructions vary by patient and by the specific hospital protocol, so treat the following as general orientation, not a substitute for what your own care team tells you:

  • You'll typically be asked to fast for several hours beforehand
  • Bring a complete list of current medications, including any blood thinners — your doctor needs to know about these well in advance, since some may need to be paused
  • Arrange for someone to drive you home; you won't be cleared to drive yourself afterward
  • Expect a short recovery period focused on the puncture site, with some activity restrictions for a day or two
  • Ask your doctor in advance what to expect regarding results — sometimes findings are discussed immediately, sometimes at a scheduled follow-up

How doctors diagnose heart disease more generally

Beyond these three tests, a diagnostic workup typically starts with the basics — a detailed symptom history, a physical exam, blood pressure, a resting ECG, and blood tests including a lipid panel — before advancing to stress testing, echocardiography, or catheterization as indicated. It's a stepwise process by design: each test is chosen based on what the previous one suggested, rather than every patient receiving every test regardless of relevance.
